Making A Life Transition:
On Campus and Community Living

Let New Hope help you achieve your dreams. Our services for adults focus on gaining independence, achieving goals and providing opportunities for choice. Consider New Hope if you or a family member has an intellectual or developmental disability or brain injury. We recognize the specialized, and often complex, needs of individuals and can assist you on your path to living a rich and meaningful life.

Service Acquisition / Eligibility

  • New Hope Residential Services serves primarily those with a diagnosis of:
    • Intellectual Disability
    • Developmental Disability
    • Brain Injury
  • All applicants are considered on the basis of his/her individual need.
  • No individual will be denied services based on race, color, national origin, sex, age or sexual orientation.
  • We request for proof of disability.
  • Work Services serves those listed above needing vocational rehabilitative services.
  • Must not exhibit severe emotional, psychological or behavior problems; must not display severe aggressive, violent or destructive behaviors or behaviors that are criminal in nature. Behaviors must not be detrimental to self or others.
  • Applicants must have third party sponsorship for funding or be able to pay for services.

Application/Admission Process For Residents of Iowa

  • Complete the application linked here, or contact Jenna Brown, Admissions Coordinator, (712) 792-5500 or [email protected].
  • Return application, along with other requested information to SDC
  • Application information will be reviewed by New Hope’s Admissions Committee to determine ability to meet needs. A meeting with the individual to gather more information may be requested.
  • The applicant will be notified of approval or denial for waiting list. If approved, the individual will be considered for vacancies when they arise or when services can begin.
  • The approved applicant should notify his/her county of legal settlement to inform them of his/her intentions for services at New Hope.
  • When accepted for services, a current psychological evaluation and physical examination will be required prior to admission for Residential Services.
  • When a vacancy is accepted by the applicant, an admission date will be set. Necessary approvals for licensures and funding will be secured.

 

Application/Admission For Out of State Applicants

A person interested in applying for services, but is not a resident of Iowa, must have third party sponsorship for funding, be able to pay for services privately, or establish Iowa residency and qualify for Iowa Medicaid.

To apply for Iowa Medicaid, call the DHS Contact Center at 1-855-889-7985 between the hours of 7:00 a.m. 6:00 p.m. Monday through Friday. To download and print an application or complete an online application visit the DHS Services Portal.
